Philips Recalls Lightolier Glass Accessory over Breaking Hazards

Hazard: The decorative glass lens can shatter and result in falling pieces of glass, posing a risk of laceration to the consumer.

Lightolier Glass Accessory (AF4SY)

Incidents/Injuries: Philips has received one report of the glass lens breaking. No injuries have been reported

Description: This recall involves Philips “Lightolier” brand AF4SY decorative glass lenses. This clear glass lens, used to “soften” light with an additional lens layer, is 4 ¾ inches in diameter and 1/8 inch thick.  The lenses were manufactured between January 2007 and December 2013. There are no markings on the product. The product number is on the packaging.

Remedy: Consumers should immediately remove the recalled glass lens accessory and contact Philips Lighting at 1-800-475-9840, via email at glassaccessory@philips.com for a free replacement unit.
